Year 8 - Human Papillomavirus (HPV) Immunisations - 23rd May 2024

The Buckinghamshire Immunisation Service will be visiting AVA on Thursday 23rd May 2024 to offer the HPV vaccination to both boys and girls aged 12 – 13 years old (School Year 8) as part of the National Childhood Immunisation Schedule.

This is now a single dose programme that if received between the ages of 12 - 13 years provides up to 87% protection against HPV related cancers and 97% against pre-cancerous cells.

Consent for vaccinations

All parents are requested to complete a form giving consent or indicating the reason for declining as soon as possible. (Please choose Year 8 when completing the consent form).         

This year all completed consent forms (yes or no consents) will be entered into a prize draw for the opportunity to win a £75 Amazon voucher.

Please complete the electronic consent form (one for each child) via HPV Vaccination Consent Form (buckschildimmunisations.co.uk) or the QR Code above and insert your school’s unique code: EE135879

This form must be submitted by 9am, one full school day before your session to ensure your child receives their vaccinations on the day.  If you consent after this date your child may not be able to receive their immunisations on the day and a link will be sent to you offering an appointment at one of our catch-up clinics based in Aylesbury, Amersham and Wycombe.

We record vaccine refusals from young people and parents to ensure health records are up to date.  If you decide that you do not wish for your child to be vaccinated, please select NO consent, giving the reason. This will help us plan and improve the service.
