The Buckinghamshire Immunisation Team will visit Aylesbury Vale Academy on Wednesday 11 November 2026 to administer flu vaccinations to students.
Parents and carers are asked to complete an online consent form whether they wish to consent to or decline the vaccination. Forms must be submitted at least one full school day before the vaccination date.
To complete the form:

Visit the Flu Vaccination Consent Form.

Enter the Academy's unique school code: EE135879.

Select your child's current year group and complete the form.
A separate form must be submitted for each child.
